The systems own the brand queries. They do not own "family doctor near me" in a specific ZIP code.

Demand splits into submarkets. The Inner Loop (the Heights, Montrose, River Oaks, West University) is the cash-pay and PPO corridor for dermatology, concierge primary care, and med-spa work. The practical read: inside the Loop you compete on reviews, subspecialty content, and pricing transparency. In Katy or Cypress you can take the map pack with a fraction of that effort. Where the office sits should shape the entire program.

By fighting where size doesn't help. System domains win branded and informational searches, but the map pack ranks on proximity, profile quality, and reviews, three things a single location controls completely.

AI Overviews favor pages that answer a specific question with a specific fact. Publish your actual self-pay prices, physicians, and same-week availability and you become quotable; a system's generic service-line page isn't.

The other lane is the long tail. "Endocrinologist that treats Hashimoto's Houston" has real volume and almost no dedicated pages. Systems don't write that page. You can.
